The Aldershot Programme
Aldershot Centre for Health serves a diverse and dynamic population, many of whom face ongoing lifestyle-related health concerns such as poor diet, low energy, emotional eating, and preventable long-term conditions, including Type 2 diabetes, obesity, and high blood pressure.

In an area where clinical teams are under increasing time and resource pressure, this coaching scheme provides a bridge between medical advice and patient action, offering tailored, one-to-one support that helps individuals turn good intentions into healthy habits.

This programme is designed to:

Relieve pressure on GPs and nurses by offering continued support outside appointments, reducing repeat visits related to lifestyle-based issues

Strengthen patient engagement by helping individuals understand their health, build confidence, and feel capable of managing their choices.

Support NHS strategic goals by placing prevention, empowerment, and person-centred care at the core of delivery.

Foster long-term change by engaging not only patients but their families, creating healthier home environments that promote shared wellbeing
This is a low-risk, high-impact intervention that can be easily integrated into your existing team structure with minimal disruption and a strong potential for measurable outcomes.
